Employee Health

Before Your Appointment 
  • You will receive an email from noreply@prismahealth.org within 24 hours of your offer. If your start date is more than 100 days away, you will receive this email once it is within 100 days. Please monitor your email inbox, including your junk folder.
  • Follow the instructions in the email to activate your applicant portal through Enterprise Health.
  • Schedule your Employee Health visit as soon as you receive the email link. You may choose any location from the drop-down list that is convenient for you.
  • To ensure medical clearance for your start date, your screening must be completed at least 10 calendar days before your scheduled start date.
  • Once your appointment is scheduled, you will receive additional instructions on how to prepare for your visit, along with directions to your screening location.
  • If you experience issues accessing the applicant portal or scheduling your appointment, please contact Employee Health at employeehealth@prismahealth.org or call 864-455-8994. Employee Health is available Monday-Friday, 7:30 am to 4 pm.
  • Please plan to attend your appointment as scheduled. Missed or rescheduled appointments may delay your start date. 
What to Bring
  • Complete Form I-9 included with your onboarding paperwork prior to your appointment.
  • Complete the paperwork in the Employee Health Portal(Enterprise).
  • Bring acceptable I-9 identification with you. Appointments cannot proceed without proper identification and may need to be rescheduled. A list of acceptable documents can be found here.
  • Bring bottles of any prescription medications that you currently take. 
  • Please bring hard copies of your immunization records and/or tests if you were unable to upload them to the Employee Health portal. If you do not have any documentation, a blood test will be drawn.
  • If you've had a positive TB test in the past, bring documentation of the positive results, chest x-ray, and/or documentation of treatment. 
Instructions for Your Appointment
  • Please do not attend your appointment if you are ill. You can cancel and reschedule your appointment through the Employee Health portal.
  • Arrive on time. If you are more than 15 minutes late, Employee Health may need to reschedule your appointment.
  • Please come to your appointment on your own. Children, family members, and friends are not permitted to accompany you to this appointment.
  • Do not wear scented lotions or perfumes to your appointment.
  • Plan to be at the appointment for at least one (1) hour.
  • Be prepared to take your official Prisma Health identification badge picture.
